Where are my old medical records and how can I get a copy of them?
All of my records prior to 2002 are at Reed Eye Associates. Per New York State Law, you may obtain a copy by requesting it in writing. This copy can be sent to you, to me, or to any other health provider that you request and must be released within 10 days. If you have had photos taken of your eyes in the past, please be sure that you specifically request copies of the photos. In general, I recommend that the copy be sent to you. That way you will know whether it was sent out.
Can I get a prescription for contact lenses?
The prescription I give you is for glasses. A contact lens prescription is different, in that it includes lens diameter, curvature, power (which may be different from your glasses), and specific lens type. These parameters are determined by your contact lens fitter (typically an optician or an optometrist), who can then write a contact lens prescription for you.
Where can I get my glasses made?
You can get your glasses made anywhere that you like. My general recommendation is that you continue to patronize any optician with whom you have had a good relationship in the past. If you are looking for a new optician, I would be pleased to offer suggestions. In general, I recommend that you select an office after you ask yourself the questions that follow. Am I willing to travel to this location for the services I want? Can I work with the same optician every time I come in? Is the office focused on the business of eyewear and willing and able to get me anything I need? Does the office meet my needs with regard to choices and styles of lenses and frames?
What are your financial policies?
I require payment of office fees at the time of service and accept payment by cash or check. If I have to contact you to pay a bill, I will add a monthly billing fee to your charge. A charge may be made for broken appointments if I have not been notified 24 hours in advance.
